Person-centered support, close to home.
Building independence. Creating real connection.
Holley Hearts Community Services, LLC provides community engagement for individuals with intellectual and developmental disabilities. We also provide transitional housing across Hampton Roads, offering a home to individuals as they work toward independent living.
Support built around the individual.
Holley Hearts Community Services, LLC delivers Medicaid-funded developmental disability services throughout Hampton Roads. Every service plan is built around the individual's own Individual Support Plan (ISP) — their goals, preferences, and pace toward greater independence — and delivered by trained direct support staff.
Three ways we support independence.
DD Waiver Services
Holley Hearts provides Medicaid-funded supports under Virginia's Developmental Disability Waivers (CL, BI, and FIS), tailored to each individual's approved Individual Support Plan and monitored for quality and DBHDS compliance.
Community Engagement
Structured, community-based activities that help individuals build social, vocational, and daily-living skills while participating meaningfully in community life — matched to each person's own interests and goals.
Transitional Housing
Supportive housing paired with hands-on skill-building — budgeting, daily living, community navigation — helping individuals move toward more independent living, with support scaled down as skills and confidence grow.
More than a service — a place to belong.
Holley Hearts' Community Engagement program is built around small-group, community-based activities that help each person build skills, gain confidence, and form real relationships close to home — not inside a facility, but out in the community itself.
Personalized Support
Support plans built around each individual's own goals, preferences, and pace — never one-size-fits-all.
Community Integration
Outings and activities out in the community, so participation feels like everyday life — not a program.
Skill Development
Hands-on practice in social interaction, independent living, communication, and workplace-readiness skills.
Relationship Building
Structured opportunities to build friendships with peers and connections with the wider community.
Confidence & Self-Esteem
A supportive, encouraging environment where progress — big or small — is recognized and celebrated.
Individuals and families across Hampton Roads.
We serve individuals with intellectual and developmental disabilities who are enrolled in — or working toward enrollment in — a Virginia DD Waiver. Support plans are built with the individual, their family or guardian, and their support coordinator, and reviewed regularly to make sure they still fit.
Person-Centered
Every plan starts with the individual's own goals, preferences, and routine.
Licensed & Accountable
DBHDS-licensed, with documented policies, staffing plans, and ongoing compliance monitoring.
Community-Rooted
Based in Hampton Roads, with staff who know the local providers and resources.
